Placing Orders:
Health care facilities, approved distributors, and approved tissue dispensing intermediaries are permitted to order allograft tissue from LifeNet Health.
Orders may be placed by phone (888) 847-7831 and/or fax (888) 847-7832. A valid purchase order number must be provided for all orders. All phone calls to LifeNet Health's Client Services Department are recorded for quality control purposes.
SWAI:
Skin and Wound Allograft Institute must be set up as an approved vendor. All purchase orders and checks must be made payable to Skin and Wound Allograft Institute.
